Abstract:	การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาต้นทุนต่อหน่วยงานการพยาบาลผ่าตัด จำแนกตามสาขาการผ่าตัดและต้นทุนกิจกรรมการพยาบาลผ่าตัดตามระยะเวลาการผ่าตัด งานห้องผ่าตัดโรงพยาบาลสุรินทร์ ทำการศึกษาต้นทุนต่อหน่วยโดยเก็บรวบรวมข้อมูลย้อนหลังในปีงบประมาณ 2557 กลุ่มตัวอย่างคือ ข้อมูลต้นทุนค่าแรง ต้นทุนค่าลงทุนและต้นทุนค่าวัสดุที่ใช้ในการจัดบริการผู้ป่วยผ่าตัดในปีงบประมาณ 2557 และการศึกษาต้นทุนกิจกรรมการพยาบาลผ่าตัดด้วยการสังเกตการปฏิบัติกิจกรรมการพยาบาล ผ่าตัด กลุ่มตัวอย่างคือ พยาบาลห้องผ่าตัดจำนวน 44 คนและจำนวนผู้ป่วยทั้งสิ้น 358 ราย จากการผ่าตัดของ 9 สาขา ระยะเวลาในการเก็บรวบรวมข้อมูลระหว่างเดือนมกราคม ถึงเดือนเมษายน พ.ศ.2557 เครื่องมือที่ใช้ในการวิจัยมี 3 ชุด ประกอบด้วย ชุดที่ 1 แบบบันทึกข้อมูลต้นทุนต่อหน่วยบริการ ชุดที่ 2 พจนานุกรมกิจกรรมการพยาบาลผ่าตัด งานห้องผ่าตัด โรงพยาบาลสุรินทร์ และชุดที่ 3 แบบบันทึกข้อมูลต้นทุนกิจกรรมการพยาบาลผ่าตัด เครื่องมือทั้ง 3 ชุดผ่านการตรวจความตรงตามเนื้อหา และเครื่องมือชุดที่ 3 แบบบันทึกปริมาณเวลามีค่าความเที่ยงเท่ากับ .90 ผลการวิจัยพบว่า ต้นทุนต่อหน่วยงานการพยาบาลผ่าตัด โรงพยาบาลสุรินทร์ มีมูลค่าต้นทุนเฉลี่ย เท่ากับ 4,126.51 บาท/ราย โดยต้นทุนรวมทางตรงมีอัตราส่วนของต้นทุนค่าแรง : ค่าลงทุน : ค่าวัสดุ คิดเป็นร้อยละ 29.19 : 40.08 : 30.73 ต้นทุนต่อหน่วยบริการที่มีค่าสูงสุดคือ การผ่าตัดสาขาศัลยกรรมประสาทมีค่าเท่ากับ 15,827.14 บาท/ราย และต้นทุนต่อหน่วยบริการที่มีค่าต่ำสุดคือ การผ่าตัดสาขาสูติ-นรีเวชกรรมมีค่าเท่ากับ 2,040.87 บาทต่อราย ต้นทุนกิจกรรมการพยาบาลผ่าตัดเฉลี่ยเท่ากับ 1,230.80 บาท/ราย และเป็นต้นทุนกิจกรรมก่อนผ่าตัดเฉลี่ยเท่ากับ 143.61 บาท/ราย ต้นทุนกิจกรรมการพยาบาลระยะผ่าตัดเฉลี่ยเท่ากับ 909.80 บาท/ราย และต้นทุนกิจกรรมการพยาบาลระยะหลังผ่าตัดเฉลี่ยเท่ากับ 177.39 บาท/ราย ทุกสาขาการผ่าตัดมีต้นทุนกิจกรรม ในระยะผ่าตัดมีค่าสูงสุด รองลงมาคือ ระยะหลังผ่าตัดและระยะก่อนผ่าตัดมีต้นทุนต่ำสุด
Other Abstract:	The purposes of this study were to determine the unit cost of operations and perioperative nursing activities cost during the perioperative time at Surin Hospital. Primary and secondary data were used in this study. The primary data concisted of, the data of observing 44 operative nurses and the data of 358 operative cases, from January to April, 2014. The secondary data concists of the, financial and material department costs of Surin Hospital from October 1, 2013 to September 30, 2014. There were 3 sets of instruments used including 1) the record relating to the analysis of unit costs, 2) perioperative nursing activities dictionary of the operation department at Surin Hospital, and 3) the records related to the analysis of perioperative nursing activities cost. All instruments were tasted for content validity and reliability and the reliability of the observation was .90. The major finding were as follows: 1. The unit cost of the operating service room was 4,216.51 baht per case. The ratio of direct cost devided between labor cost, capital cost and material cost was 29.19 : 40.08 : 30.73 percent. The hightest unit cost were neurological cases at 15,827.14 baht per case and the lowest unit cost were obstetric-gynaecological cases at 2,040.87 baht per case respectively. 2. The average of perioperative nursing activity costs was about 1,230.80 baht per case respectively. Preoperative nursing activities costs were about 143.61 baht per case respectively. Intraoperative nursing activities costs were 909.80 baht per case respectively. Postoperative nursing activities costs were 177.39 baht per case respectively. Intraoperative nursing activities costs were the highest and postoperative nursing activities costs were higher than preoperative nursing activities.
